Please print out and complete the attached order form (see bottom of page) and put it, with a cheque payable to "Chorley Marlins A.S.C", in a sealed envelope in the Chorley Marlins letterbox by the Noticeboard at All Seasons Leisure Centre. Please address the envelope "FAO Swim Kit Manager". Delivery will be 1-4 weeks.
